To my patients and families,
I would like to thank all my patients and families for the trust you have given me over the years as your physician. Taking care of your family has been an honor for my staff and me. Your continuing wellness and health are our top priorities.
I have had to make the difficult decision to close my practice due to personal reasons, effective as of 12/22/22. I will have limited availability until that date for my patient’s urgent care needs.
It is important that you continue with appropriate medical care; therefore, you should establish contact with another physician in our practice. If you wish to see a provider at another practice, please send us that information as soon as possible.
We are also sending out letters with additional information to my patients. Please feel free to ask the clinic staff if you have any questions on continued care.
